Circumcision in children and men
The desire for circumcision may be religiously motivated, medically indicated, or — increasingly often — simply for hygienic reasons.
When a child is to be circumcised, it is extremely important that this once-in-a-lifetime procedure is not left to an unqualified practitioner, but is always performed by a specialist in surgery, pediatric surgery, or urology.
As an adult, one naturally wants the procedure to be painless, performed on an outpatient basis, and in a private, discreet atmosphere.
Experienced pediatric surgeons, general surgeons, and urologists guarantee a completely pain-free circumcision and deliver aesthetically and functionally optimal results.
In Vienna and most parts of Austria, circumcision — even for medical reasons — is hardly possible within the first year of life in private clinics or public hospitals if only supplementary insurance is available. For exactly these cases, we have our own dedicated procedure room where infants can be operated on completely pain-free under local anesthesia without any problems.

When should my son be circumcised?
This question is frequently debated. On the one hand, performing a technically perfect circumcision on a very small boy is more challenging and carries a higher risk of penile injury; on the other hand, early intervention is better for healing and for the child itself.
In Jewish families, circumcision is required on the eighth day of life; in Muslim families, the timing is not strictly defined. In the United States, most boys are circumcised within the first few days after birth.
In Austria, renowned paediatricians recommend not performing circumcision before the third day of life, as coagulation problems may still occur earlier. Consideration must also be given to the mother’s condition after delivery — in particular, the procedure should not take place if she is experiencing a postpartum depressive phase.
Generally speaking, if circumcision is being performed in childhood for religious reasons, it should ideally take place within the first week or first month of life.
If this is not possible for various reasons, we recommend performing the circumcision no later than the end of the fourth month of life, because after that age it becomes increasingly difficult to perform the procedure completely pain-free and anxiety-free without anesthesiological support.
In cases of medical indication, the timing is straightforward: once the indication exists, the circumcision can be planned without urgency, regardless of age.

For pregnant women
Delivery in a private clinic — circumcision shortly after birth
If you give birth in a private clinic (e.g. Privatklinik Döbling, Goldenes Kreuz, Rudolfinerhaus, etc.) and would like Prof. Paya to perform your son’s circumcision during your hospital stay in the first few days after birth, this can be arranged without any problem.

What does circumcision mean for a newborn/infant?
Infants most certainly feel pain just as intensely as adults. It is a dangerous misconception that a tiny baby “doesn’t feel anything anyway” and that circumcision can therefore be performed without proper pain relief.
Only in the very first days after birth is pain perception still significantly reduced — this changes quickly.
If a boy is circumcised without adequate anesthesia, he experiences excruciating pain but is unable to defend himself. This can lead to post-traumatic stress disorder — figuratively speaking, a “scar on the psyche” — which may later manifest in adolescence or adulthood as mild to severe consequences.
Possible long-term effects include unexplained pain conditions, heightened sensitivity, anxiety, lack of trust and attachment to parents, potential sexual problems or deviations, increased risk of addiction, aggression, etc.
For this reason, adequate pain and anxiety management must always be ensured during circumcision.

Methods of circumcision
Worldwide, up to 300 different techniques exist; five to six have proven sufficiently safe and practical.
Plastic ring method (Plastibell, plastic bell, etc.)
Gomco clamp method — the predecessor of the plastic ring technique
Stapler — a single-use device for particularly quick circumcision; currently still expensive (the device is imported, approx. €200). (Only for adults)
Surgical (open) circumcision — a technically demanding method that requires more time and experience, but is probably the safest method and the only one that allows precise determination of the amount of foreskin to be removed beforehand.

Costs of circumcision
Health insurance in Austria generally covers the costs of circumcision only when a clear medical indication exists.
Otherwise, the attending physician must bill privately for his/her services as well as those of the anesthetist and the entire operating team. In our clinic, these costs range — depending on the method, age, etc. — from approximately €700 to €1,500. Please refer to our separate cost page for details.
Our fee always depends on the complexity and type of circumcision. After the initial consultation and examination, we can provide a precise cost estimate.
If you have supplementary private health insurance, we strongly recommend obtaining a coverage confirmation in advance.
We always recommend a preparatory consultation a few days or weeks before the scheduled circumcision date. This initial consultation usually lasts about one hour and is billed separately.
